Supporting the health, safety and performance of Indiana Tech student-athletes.

Supporting Warrior Student-Athletes

Indiana Tech's athletic training services are provided through Parkview Sports Medicine, one of the area's leading sports medicine providers. Our athletic training team works to provide Indiana Tech student-athletes with the care and support they need throughout their athletic careers at Indiana Tech.

Pre-Participation Requirements

Sports Medicine Forms

To be eligible to participate in Indiana Tech Athletics, student-athletes must complete the required pre-participation Sports Medicine forms through Spry. New student-athletes must also complete a pre-participation athletic physical.

Information regarding Spry access and login instructions will be provided prior to the beginning of the academic year.

Required forms include:

  • Student Demographics
  • Acknowledgment of Risk Consent
  • Primary Insurance Information
  • Photos of Insurance Card
  • Pre-Participation Physical – Upload
  • Returning Student Medical Questionnaire

Student-athletes will also be asked to complete the Parkview Sports Medicine HIPAA / Medical Release and Consent to Treat Form.

Athletic Training Locations

Fitness Center

Located inside the Fitness Center, adjacent to the Schaefer Center gym.

Warrior Athletic Center

Located inside the gymnasium of the Warrior Athletic Center.

Warrior Park

Athletic training services are also available at Warrior Park for student-athletes and teams utilizing the facility.

Drug Free Sport AXIS

Education & Prevention

Indiana Tech has partnered with Drug Free Sport AXIS to provide education and prevention resources related to performance-enhancing and street drugs, proper diet and supplements.

Drug Free Sport also supports Indiana Tech's drug testing program and drug testing at NAIA National Championships.
