Indiana Tech Sports Medicine

Indiana Tech's athletic training services are provided through Parkview Sports Medicine, one of the leading health-care providers in the area.


To be eligible to participate in Indiana Tech athletics, every student athlete must complete their pre-participation Sports Medicine forms on Front Rush including a pre-participation athletic physical, if they are a new student athlete. A link with information on how to login into Front Rush and access will be sent mid-summer, prior to the beginning of the school year. The pre-participation forms on Front Rush will include:
  • Student Demographics
  • Acknowledgement of Risk Consent
  • Primary Insurance Information
  • Photos of Insurance Card
  • Pre-Participation Physical - Upload
  • Returning Student Medical Questionnaire
Student athletes will also be asked to complete the Parkview Sports Medicine HIPAA/Medical Release and Consent to Treat Form that will be sent at the same time as the Front Rush access. 

The Indiana Tech Athletic Training offices are located inside the Fitness Center, adjacent to the Schaefer Center gym. There are also athletic training offices in the Warrior Athletic Center and Warrior Park to serve student-athletes and teams that utilize those facilities. Indiana Tech has partnered with Drug Free Sport AXIS to provide education and prevention on performance-enhancing and street drugs as well as proper diet and supplements. Drug Free Sport handles all of Indiana Tech's drug testing and also handles all drug test at NAIA National Championships. A link to Drug Free Sport Axis is above.
